This is a 5-patient VITALS LOG to help CNAs/PCTs organize their rounds.

HOW IT WORKS:
Step 1: Add item to your shopping cart & check out per usual.
Step 2: Once payment is processed, you will receive an order confirmation email. Within your email receipt, there is a button called "View content" -  it will take you to the product content page. If you have not received an email receipt, please check your Spam folder. If you still can't locate it, please contact us.
Step 3: Before being redirected to the download page, you might be prompted for your email address. Simply enter the email you used at checkout, and you should be able to access the download page.
